The Hidden Deficiency Behind Fatigue, Brain Fog, and "Normal" Labs-Lynchburg, Virginia

Can we talk about the impact of how your Iron, Ferritin, and Thyroid effects on your mental health?

You wake up exhausted despite a full night's sleep. Your energy is low, your thinking feels sluggish, mentally unmotivated, unable to focus or pay attention to task, mental crowding, and you struggle to get through the day. You've gained weight, feel cold more often than others, and your hair seems thinner than it used to be.

Your doctor checked your thyroid -TSH only, and told you everything was normal.

So why do you still feel awful?

The answer may be hiding in a "Normal lab" value that many providers overlook: your iron stores and comprehensive thyroid panel.

Healthy Iron Stores Does More Than Prevent Anemia

Most people associate iron deficiency with anemia but not with cognitive and energy health. But iron is involved in hundreds of critical processes throughout the body, including energy production, brain function, neurotransmitter production, hormone regulation, and thyroid health.

Long before anemia develops, low iron stores can begin affecting how your body functions.

One of the most important — and least recognized — effects of iron deficiency is its impact on thyroid hormone production.

Your Thyroid Cannot Function Properly Without Iron

Your thyroid gland relies on an enzyme called thyroid peroxidase (TPO) to produce thyroid hormones.

What many people don't realize is that TPO is an iron-dependent enzyme. Without adequate iron, the thyroid simply cannot manufacture hormones efficiently.

Research shows that declining iron stores are associated with lower levels of Free T4 and Free T3, even when TSH remains normal and changes in thyroid antibodies that can be reactive in the absence of negative TSH or T4. This means many people may experience symptoms of low thyroid function despite being told their thyroid labs are "fine."

Iron is also required to convert T4, the inactive storage hormone, into T3, the active hormone your cells actually use.

When iron is low:

  • Thyroid hormone production decreases

  • T4-to-T3 conversion becomes less efficient

  • Cellular metabolism slows- weight gain, cognitive, mental and physical slowness

  • Symptoms of hypothyroidism can develop before TSH shows changes.

Mental Health Symptoms Often Overlap With Thyroid Disease

Many symptoms of iron deficiency are similar to many mental health symptoms as they points to those of hypothyroidism:

  • Chronic fatigue

  • Brain fog

  • Difficulty concentrating

  • Weight gain

  • Feeling cold-physically numb

  • Low motivation

  • Depression

  • Slowed recovery after exercise or movement

Because these symptoms overlap so closely, iron deficiency—and in some cases subclinical thyroid dysfunction—can easily be overlooked. Instead, symptoms such as fatigue, low motivation, brain fog, and low mood may be attributed solely to depression and treated accordingly. While depression is a very real condition that deserves appropriate care, an underlying iron deficiency or impaired thyroid function may leave the brain and body operating in a metabolically sluggish state. As a result, individuals may continue to struggle despite treatment because the cellular processes responsible for energy production, neurotransmitter synthesis, and hormone regulation have not been fully addressed.

Why Standard Thyroid Testing May Miss the Problem

Many thyroid evaluations only include TSH.

While TSH is useful, it does not tell the whole story.

Someone can have:

  • Normal TSH

  • Low iron stores

  • Reduced thyroid hormone production

  • Poor T4-to-T3 conversion

  • Significant symptoms

Yet still be told their thyroid is normal.

This is why evaluating iron status alongside thyroid function is so important.

What Iron Tests Matter Most?

A complete iron assessment may include:

  • Ferritin

  • Serum Iron

  • Total Iron Binding Capacity (TIBC)

  • Transferrin Saturation

  • Complete Blood Count (CBC)

  • Zinc

Ferritin is particularly important because it reflects stored iron. Many individuals experience symptoms long before anemia develops.

A Root-Cause Approach to Fatigue, Brain Fog, Low Mood

When someone struggles with fatigue, brain fog, low mood, or symptoms that resemble depression, simply treating symptoms may not be enough.

The more important question is:

Why isn't the body functioning optimally?

Sometimes the answer isn't another medication.

Sometimes it's identifying and correcting a correct cellular dysfunction or blocked pathways that has been affecting normal cellular functions and cellular energy.

At Axxiums, we take a root-cause approach that evaluates metabolic, hormonal, and nutritional factors that influence both physical and mental health.

If you've been told your labs are normal but still don't feel like yourself, it may be time to look deeper.
