When Hormones, Attention, and Emotional Overwhelm Start to Look the Same
Many teenage girls are told they are:
“too emotional”
“lazy”
“dramatic”
“disorganized”
“overreacting”
“not trying hard enough”
But sometimes what looks like a behavior problem is actually an untreated medical, hormonal, or neuropsychiatric condition.
And sometimes it is more than one condition happening at the same time.

PMDD (Premenstrual Dysphoric Disorder)-with menstrual cycle onset

These conditions can look incredibly similar — especially during puberty — and many girls go years without proper diagnosis because their symptoms are dismissed as “normal teenage hormones.”

Teen girls with PMDD, ADHD, or anxiety may all experience:

PMDD symptoms are tied to the menstrual cycle.
Symptoms typically:
begin 7–14 days before a period
worsen in the late luteal phase
dramatically improve once bleeding starts
repeat monthly
Many teens tries to hid their symptoms but often describe feeling like:
“I become a completely different person before my period.”
Common PMDD Symptoms in Teens
Emotional Symptoms
sudden rage or irritability
crying spells
increase emotional sensitivity
hopelessness
Wanting to self Isolate- at home and socially
Moodiness
panic symptoms
feeling emotionally “raw”
Cognitive Symptoms
brain fog
trouble concentrating
low motivation
memory issues
Physical Symptoms
bloating
headaches
fatigue
breast tenderness
food cravings
insomnia or oversleeping
low motivation or energy
changes in heart rate and respiration
The Key Clue:
PMDD is cyclical.
There is usually a symptom-free or much calmer window after the menstrual cycle begins.
That predictable hormonal pattern is one of the biggest clues.

ADHD in Teen Girls: The Often-Missed Presentation
ADHD in girls is frequently missed because it does not always look hyperactive.
Many girls with ADHD are:
internally overwhelmed
inattentive
emotionally dysregulated
perfectionistic
anxiousness
seeking validation
fear of messing up (in various aspects of life or social interactions)
chronically exhausted from masking symptoms
Common ADHD Symptoms in Teen Girls
chronic procrastination—or conversely, feeling a desperate need to finish a task immediately when unsupported—which drastically increases frustration and irritability.
difficulty starting tasks
forgetfulness
losing things
time blindness
emotional impulsivity
rejection sensitivity
Inconsistent school performance; many girls thrive early on but begin to struggle in middle or high school as academic demands increase and structure and systems decreases.
hyperfixation
an internal sense of constant mental busyness, leaving them feeling moderately overwhelmed in almost every area of life.
Many girls with ADHD are not diagnosed until puberty, when hormonal changes increase executive functioning difficulties.
Important Hormonal Connection:
Estrogen strongly affects dopamine.
During the luteal phase, the normal drop in estrogen levels can impair dopamine signaling, worsening focus and emotional regulation.
This means many girls with ADHD experience:
worse focus
more emotional dysregulation
increased overwhelm
worsening sensory sensitivity
more impulsivity before their period
Some girls who think they “only have PMDD” may actually have underlying ADHD that becomes dramatically worse during hormonal shifts.
Anxiety Disorders in Teen Girls
Anxiety is also extremely common during adolescence.
Unlike PMDD, anxiety symptoms are usually more persistent across the month rather than strictly cyclical.

chronic overthinking
panic attacks
muscle tension
stomach aches
perfectionism
excessive worry

avoidance behaviors
social anxiety
fear of failure
rumination
Teens with anxiety may appear:
highly responsible
perfectionistic
emotionally sensitive
people-pleasing
while internally feeling constantly overwhelmed.
PMDD vs ADHD vs Anxiety: The Biggest Differences

The Reality: Many Girls Have More Than One
This is where things become more complex.
Many adolescents with PMDD also have:

autism spectrum traits
Hormonal fluctuations can amplify all of these underlying vulnerabilities.
Why Puberty Changes Everything
Puberty is not just a hormonal event.

Estrogen and progesterone affect:

The Hidden Medical Contributors That Can Mimic or Worsen Symptoms

Treatment depends on the underlying drivers and may include:

SSRIs when appropriate

For some teens, treatment is life-changing once the correct diagnosis is finally recognized.
The Bottom Line
Not every emotional teenage girl has PMDD.
Not every distracted teenage girl has ADHD.
Not every overwhelmed teenage girl “just has anxiety.”
